The patient is a 64 year old male with long-standing history of active dental caries and marginal periodontitis. Dental implants were placed in two sites in 1994. He has only seen his dentist on dental emergency visits in the last two years. He has been non-compliant for periodontal and dental implant recall visits. The patient continues to show dental decay and periodontal breakdown.
The radiograph shows excellent bone height and stability around the single tooth implant placed in the # 20 site. Adjacent teeth show significant decay, calculus deposits and bone loss. The dental implant appears to have a better prognosis than his natural teeth.
Message to consider-
Dental implants may be a better restorative option for patients with aggressive dental caries rate. Proper treatment planning, however, should include education on the need for regular implant follow-up care and regular dental and periodontal care. Dental implants need to be placed in the healthy mouth. They need routine personal and professional care, as do the natural teeth. This patient is fortunate. Despite poor personal dental hygiene and minimal follow-up care, the implant shows great stability and all parameters of health.
